Smiths Falls – Lanark Leeds and Grenville Addictions and Mental Health (LLGAMH) is pleased to announce the launch of the Lanark Leeds and Grenville HART Hub’s Lanark site, which will officially begin welcoming clients on Monday, December 8. The opening of the Leeds and Grenville site is scheduled to follow in early spring 2026.

A provincial initiative funded by the Ministry of Health, the Lanark Leeds and Grenville HART Hub will operate out of two locations to provide community members with low-barrier access to a full continuum of addictions and mental health support for long-term recovery and, in some instances, housing stability.

“The opening of the HART Hub in Lanark County marks a meaningful step forward in connecting people to the care they need, close to home,” said Vijay Thanigasalam, Associate Minister of Mental Health and Addictions. “As part of Ontario’s network of 28 HART Hubs, this site will help deliver integrated, community-based services that support recovery. Together with our community partners, we are creating a safe, supportive space where people can access the care they need, when they need it, on their path to recovery. This is about restoring hope, strengthening communities and helping people build stability.”

“The HART Hub in Smiths Falls is a welcome addition to health services in Lanark County,” said John Jordan, MPP for Lanark-Frontenac-Kingston. “This funding represents vital partnerships and collaboration between the Province, Lanark, Leeds and Grenville Addictions and Mental Health and over 50 community organizations. The HART Hub will bring together mental and primary health services, transitional housing and social supports, all under one roof. It’ll not only give some of our most vulnerable residents a way out of crisis, but a pathway toward stability and hope.”

The HART Hub’s Lanark site will focus on providing clients with long-term treatment and housing support and will complement and have access to the withdrawal services offered at the Leeds and Grenville site (when it opens in spring 2026).

The Lanark site includes 17 beds divided into two key components:

  • 8 residential addiction treatment beds, offering extended treatment and recovery support for individuals continuing their rehabilitation
  • 9 transitional supportive housing beds, providing stable accommodations with wraparound services for those preparing to move toward independent living

The HART Hub’s Lanark team has begun accepting client self-referrals by phone to keep the barrier as low as possible. The HART Hub is a good fit for individuals experiencing unstable housing, a substance use disorder, and mental health challenges who are ready for recovery with a pathway to stable housing. As a provincially funded program, there is no additional cost to clients.

Shawn Pankow, Mayor, Town of Smiths Falls, shared, “The impact of substance use and addiction has weighed heavily on our community for years. It has put incredible stress on families, stolen promising futures, and taken lives. It has strained our emergency room, police and paramedic services. It has manifested despair in too many places, far too often.

“The opening of a HART Hub in Smiths Falls has given us hope. It provides those suffering through addiction access to treatment and an opportunity for a future that was previously out of reach. I am incredibly grateful to LLGAMH, MPPs John Jordan and Steve Clark and the province for making this commitment towards the long-term health and wellness of our broader community.”

Led by LLGAMH, the HART Hub’s Lanark team includes clinical leadership, addictions counsellors and rehabilitation workers. Strong community partnerships will support both HART Hub sites in delivering the wraparound services key to clients’ long-term recovery.

“The Board of Directors at LLGAMH is excited about the opening of the Lanark Leeds and Grenville HART Hub’s Lanark site,” said David Price, Board Chair, LLGAMH. “We are confident and proud that our LLGAMH team understands that every person’s experience is different, and our HART Hub team will support each client along their unique recovery journey, helping them at every point to reach their goals and prepare them to rebuild their lives in the community.”

“We are honoured to welcome clients to begin their recovery journey at our Lanark Leeds and Grenville HART Hub,” said Kim Gifford, CEO, LLGAMH. “The Ontario government’s HART Hub model offers our communities low-barrier access to a full continuum of mental health and addictions care that will give individuals a real opportunity for lasting recovery and, in some cases, a pathway out of homelessness or unstable housing.”
