Three-Vehicle Collision on Highway 15 in Lombardy Leads to One Fatality

Lombardy – The Ontario Provincial Police (OPP) is investigating a fatal three-vehicle collision on Highway 15 in Lombardy between County Road 1 and County Road 5.

On Friday, May 29, 2026, at approximately 5:00 p.m., members of the Leeds County Detachment of the OPP responded to a report of a head-on collision involving two cars and an ambulance near Otter Lake Park. The ambulance was returning to base and had no patients onboard.

The driver of one of the vehicles, a 34-year-old male from Elizabethtown-Kitley, was pronounced deceased at the scene. A young child in that vehicle was airlifted to a trauma centre by Ornge air ambulance with life-threatening injuries.

The driver and passenger of the ambulance were taken to hospital with serious injuries. The driver and sole occupant of the third vehicle sustained minor injuries and was transported to hospital.

Leeds Thousand Islands Fire Service, Leeds Grenville Paramedic Service, and OPP Technical Collision Investigators and Reconstructionist assisted Leeds County OPP in response to the incident. Highway 15 was closed for several hours and has since reopened.

The Leeds Grenville Paramedic Service and the United Counties of Leeds and Grenville posted a statement today:

𝗦𝘁𝗮𝘁𝗲𝗺𝗲𝗻𝘁 𝗼𝗻 𝗛𝗶𝗴𝗵𝘄𝗮𝘆 𝟭𝟱 𝗜𝗻𝗰𝗶𝗱𝗲𝗻𝘁

“Yesterday afternoon, one of our ambulances was involved in a serious collision on Highway 15 near Lombardy.

The two paramedics involved were transported to The Ottawa Hospital – Civic Campus, where they were treated for their injuries and have since been released.

We are actively gathering confirmed information and are working closely with our partner agencies as they continue their investigation. As this process is ongoing, we will not be providing further details at this time.

We extend our sincere thanks to all emergency responders who attended the scene for their professionalism and care.
Our thoughts are with everyone involved and all those affected by this incident.”
